    Job Description

     

    Do you enjoy working with underserved populations in a supportive outpatient environment? We are looking for a passionate Family Medicine Physician to provide high-quality, trauma-informed care to the patients in our community health clinic. Successful candidates will be able to provide comprehensive medical services for patients of all age ranges. The candidate will be responsible for meeting, assessing and diagnosing patients in our clinic and providing them with healthcare services that meet the national standards of care.

    • Perform evaluation and treatment of patients in an outpatient clinic setting.
    • Participate in care team huddles, provider meetings, and all-staff meetings.
    • Ensure patient information is always secure and confidential.
    • Performs job duties collaboratively with health center management and exercises good judgement.
    • Provides excellent customer service to internal and external clients/patients by being responsive to all inquiries in a timely manner.
    • Meet productivity standards for care providers, including a goal of seeing 16-24 patients daily.
    • Serve as a clinic supervisor for mid-level providers.
    • Initiate and follow through on specialist referrals. Communicate with specialist providers when necessary.
    • Review and sign off on all laboratory and radiology results. Ensure that there is patient follow up when necessary.
    • Keep accurate, concise, and thorough documentation of all medical history, exam results, and discussions with patients in relation to their health care. Close all notes in the EMR in a timely manner.
    • Assess medical needs and develop a care plan and preventive health maintenance, assess acute needs, chronic conditions.

    Requirements:

    Medical Director

    • Job Type: Full-Time
    • Compensation: $225,000+ per year

    Job Description

     

    The position of Medical Director is responsible for monitoring and improving the level of care provided to patients. Qualified candidate must have an understanding and knowledge of medicine across a broad range of therapeutic areas, both clinically and technically. The Medical Director will work closely with the Executive Team. The Medical Director will work collegially with a team of doctors, nurse practitioners and nurses and provide direct patient services.

    • Supervise all psychiatric and medical functions and develop or modify medical policies as needed
    • Oversee pharmacy and laboratory services, assuring required regulatory standards are met

    Job Description

     

    We are seeking a Child Psychiatrist to join an organization with 20 years of experience as a Federally Qualified Health Center (FQHC) serving low income, underinsured, uninsured, publicly housed, and homeless populations in North Central Massachusetts.

    • Provide direct psychiatric patient care to a panel patients
    • Provide medication management consultation to primary care providers
    • Participate in the collaborative, integrated health care model
    • Participate in the educational activities of the Behavioral Health Department in collaboration with the family medicine faculty.
    • Participate in Multidisciplinary Reviews, including group meetings and reviewing documentation.
    • Diagnoses, manages, and appropriately refers patients with acute, episodic, or chronic illness according to department protocols
    • Orders appropriate laboratory and diagnostic tests according to established guidelines.
    • Facilitates and coordinates patient care referrals to specialists and outside agencies
    • Develops and provides educational materials and/or programs to patients, families, and other health care professionals.
    • Actively participates in the departmental quality assurance program.
    • Serves on appropriate department and/or hospital committees.
    • Supervisor physician and non-physician practitioners interested in treating children and adolescents.
    • Demonstrate understanding and commitment of the health center mission.
    • Performs other related duties as required and as assigned.
