• Perform evaluation and treatment of patients in an outpatient clinic setting.

• Participate in care team huddles, provider meetings, and all-staff meetings.

• Ensure patient information is always secure and confidential.

• Performs job duties collaboratively with health center management and exercises good judgement.

• Provides excellent customer service to internal and external clients/patients by being responsive to all inquiries in a timely manner.

• Meet productivity standards for care providers, including a goal of seeing 16-24 patients daily.

• Serve as a clinic supervisor for mid-level providers.

• Initiate and follow through on specialist referrals. Communicate with specialist providers when necessary.

• Review and sign off on all laboratory and radiology results. Ensure that there is patient follow up when necessary.

• Keep accurate, concise, and thorough documentation of all medical history, exam results, and discussions with patients in relation to their health care. Close all notes in the EMR in a timely manner.

• Assess medical needs and develop a care plan and preventive health maintenance, assess acute needs, chronic conditions.

• Conduct psychosocial assessments, formulate treatment plan, provide quality clinical services

• Open to both LMFTs and AMTFs
• 1+ years of experience with mental health treatment & working in a treatment center
• Conduct initial biopsychosocial assessment, develop treatment plan, contribute to treatment team coordination, and developing individualized aftercare plan
• Provide individual, family, and group therapy and complete all necessary documentation in accordance with ASAM requirements
• Collaborate with family and other supportive services in the community for continuity of care
• Participate in the utilization review process
• Active member of the interdisciplinary treatment team. Attend and participate in staff development programs and administrative meetings.
• Comply with all company and department policies and procedures, quality assurance, safety, environmental, infection control and complies with requirements of accreditation and regulatory agencies.

• Conduct telephonic Social Work outreach to assigned members to assess health, environment, mental health, nutrition, functionality, decline, and psycho-social areas of concerns by conducting a Social Work assessment.
• In response to assessments, coach and problem solve with member to identify and address specific goal(s) to support health and behavior change.
• Timely documents Social Work Interventions and goals in the EMR. Adheres to the departments documentation requirement of submitting Documents all telephonic and virtual visits into the medical record the same day care is provided.
• Promotes the value of Health Care Advanced Directive and documents discussion of member preferences.
• Charts thoroughly and timely on all Member encounters.
• Provide appropriate interventions to optimize health and well-being. Interventions may include education, the coordination of community- based support services, supportive counseling for mental health conditions, care navigation, advanced care and end of life planning
• Collaborate with other members of the interdisciplinary team.

• Assessing medical necessity for admission, reviewing appropriate levels of care.
• Perform initial evaluations of client needs and develop customized treatment plans.
• Complete precertification documentation upon client admission and retrospective reviews in a timely manner to ensure continuous coverage.
• Collaborate with clinicians in order to obtain necessary clinical documentation for reviews.
• Collaborate with admissions and management to determine appropriate length of stay and discharge dates.
• Reviewing all medical records before they are submitted.
• Updating spreadsheets for reporting and documentation purposes.
